A Personal Trainer can put together a structured, progressive exercise programme, tailored individually to your fitness level and your goals. He or she will be able to assess your current level of fitness, and also to help you set achievable, positive and measurable goals - wanting "to tone up a bit" sounds fine, but is rather difficult to actually measure. A trainer can translate this into meaningful information, and will be able to monitor your progress.
A good Personal Trainer will also be a first-rate motivator. If you know you have an appointment (that you've paid for!) at the gym, you're far more likely to go, and once you get there, your trainer will take care of the rest of your workout.

Finding a good Personal Trainer is often harder than it seems - it is still a relatively new industry and many individuals have set themselves up as trainers with little or no qualification to do so. Key things to look for in a Personal Trainer are 1) a recognised and comprehensive fitness/training qualification (from someone like the American College of Sports Medicine or Premier Training), 2) an up to date first aid/CPR certificate and 3) personal liability insurance.
